Follow these steps to check your organization’s HMM completion criteria:
- Manual Log in: Access HMM with your username and password credentials at https://myhbp.org/hmm12
- SSO: If your company has SSO, please sign into your VPN and then access HMM from your client specific SSO URL.
- Click on your profile icon in the upper right hand corner.
- Click on the Your Profile link.
Your Completion Criteria will display on this page.
